Plasma "ketone" level in the diagnosis and treatment of diabetic acidosis.

چکیده
Introduction In a patient suspected of having diabetic ketoacidosis it is useful to be able to rapidly confirm the diagnosis. In this situation we have used a simple side-room method for the detection of plasma ketones and now report our experience with it. Rothera (1908) described a method of identifying ketone bodies in urine which depends on the reaction between sodium nitroprusside and acetoacetic acid in the presence of free ammonium ions. Wishart (1920) adapted this method for use with plasma. We have used 'Acetest' (Ames & Co. Ltd) tablets, which are based on this reaction, as a qualitative and quantitative measurement of plasma 'ketones'. 'Acetest' tablets react with both aceto-acetate and acetone but they are more sensitive to the former, therefore the reaction is mainly a measure of the aceto-acetate concentration. The concentration of aceto-acetate correlates with the severity of diabetic ketoacidosis (Nabarro, 1962).

Diagn Invest
Diabetic ketoacidosis (DKA) is a life-threatening complication of diabetes mellitus (DM). The standard method of detection of ketone bodies is the dipstick method, which detects semiquantitatively acetoacetate, but not β-hydroxybutyrate (β-HB). The objectives of the current study were to assess the diagnostic utility of β-HB to diagnose diabetic ketosis (DK) and DKA in cats and to establish a c...
متن کاملEuglycemic Diabetic Ketoacidosis with Persistent Diuresis Treated with Canagliflozin
Diabetic ketoacidosis is characterized by hyperglycemia, anion-gap acidosis, and increased plasma ketones. After the resolution of hyperglycemia, persistent diuresis is rare. We herein report the case of a 27-year-old Asian woman with type 2 diabetes who was treated with a sodium-glucose cotransporter 2 (SGLT2) inhibitor (canagliflozin) who developed euglycemic diabetic ketoacidosis and persist...
متن کاملD-lactate: a novel contributor to metabolic acidosis and high anion gap in diabetic ketoacidosis.
Diabetic ketoacidosis (DKA),the most common and serious acute complication of diabetes, is characterized by hyperglycemia and severe high–anion-gap metabolic ac-idosis with ketonemia (1). In DKA, the high anion gap is attributed largely to excessive production of blood ketone bodies, and serum -hydroxybutyrate quantification is recommended for the diagnosis and monitoring of DKA (2). However, ...
متن کاملPediatric non-diabetic ketoacidosis: a case-series report
BACKGROUND This study is to explore the clinical characteristics, laboratory diagnosis, and treatment outcomes in pediatric patients with non-diabetic ketoacidosis. METHODS Retrospective patient chart review was performed between March 2009 to March 2015. Cases were included if they met the selection criteria for non-diabetic ketoacidosis, which were: 1) Age ≤ 18 years; 2) urine ketone positi...
متن کاملManagement of Diabetic Ketoacidosis in Children
Diabetic ketoacidosis (DKA) is one of the leading causes of mortality in diabetic children. This study aimed to determine the frequency of mortality and complications of DKA during treatment in order to make the necessary modifications with regard to the common treatment problems. This was a cross sectional study, where patients were selected by examining the records of 112 children under the ...
