چکیده مقاله:

Background: Femoral head avascular necrosis is the cause of paralyzing status of youth population. Initial diagnosisis the main element in treating the disease. Bone grafting and core decompression are the approved cures at the earlysteps of the disease. Hip replacement in a total manner is the common cure in the final stages. The optimal treatment inthe intermediate stages is partially disputable. We investigated several patients with femoral head osteonecrosis curedwith impacted cancellous allograft and open core decompression using the lightbulb technique. Methods: A total of 46 patients (58 hips) suffering from femoral head osteonecrosis were evaluated in this crosssectionalstudy. Patients were classified into two groups: A (stage 2B Ficat) and B (stage 3 Ficat) to be treated with theimpaction of cancellous allograft and by open core decompression. Radiographic results, demographic data, and rangeof hip joint motions were recorded. The patients were assessed through employing the Harris hip score (HHS) andvisual analogue scale (VAS) index prior to operation and over five years following surgery. We also studied radiographicalterations of femoral head. Results: The means of HHS and VAS were developed following the operation. Radiographic outcomes promoted inboth groups, however, it was better in group A. 12 (40%) and six (22%) hips (40%) in groups A and B, respectivelydisplayed developed stages following the operation. The hip ROM was enhanced with the mean of 15-20 degrees(p <0.005).Conclusion: Open core decompression combined with allograft impaction sounds to be influential in the developingsteps of femoral head necrosis and leads to joint discomfort and diminished pain improving ROM of the hip joint andmeanwhile procrastinating the worsening of the disease.Level of evidence: IV

Treatment of Osteonecrosis of the Femoral Head by Core Decompression , Bone Grafting , and Electrical Stimulation

Core decompression is one of the more popular procedures for the treatment of early stages of avascular necrosis (AVN) of the femoral head. However, the controversy regarding its safety and effectiveness continues. In an attempt to clarify the situation, we are reporting on a large series of cases performed by a single surgeon with long-term follow-up. This is a prospective study in which resul...

Core decompression for early atraumatic osteonecrosis of the femoral head.

We performed 41 core decompressions in 32 patients for stage I or stage II osteonecrosis of the femoral head. The intra-osseous pressure at the intertrochanteric level was raised in 28 (68%) and there was histological confirmation of necrosis in 36 hips (88%). After a follow-up of 10 to 84 months (mean 31) nine of the 12 stage I hips (75%) showed significant clinical or radiological deteriorati...

Osteonecrosis Femoral Head Treatment by Core Decompression and ILIAC CREST-TFL Muscle Pedicle Grafting

Osteonecrosis of femoral head is a painful disabling condition seen in association with many disorders like corticosteroids consumption, alcohol abuse, haemoglobinopathy (sickle cell disease, coagulopathies), certain renal, hepatic and skin disorders commonly affecting young patients ranging from 20-40 years.1-3 It is now recognized as a major musculo-skeletal problem mostly affecting the young...

Operative technique and results of core decompression and filling with bone grafts in the treatment of osteonecrosis of femoral head.

BACKGROUND The aim of this paper is to present the operative technique and results of core decompression and clearing of a necrotic zone in the femoral head followed by filling the defect with impacted autologous or synthetic bone grafts. This procedure allows for mechanical prevention of further collapse of the femoral head and loss of its spherical shape. It also creates conditions for the in...

Subchondral Insufficiency Fracture of the Femoral Head Treated with Core Decompression and Bone Void Filler Support

Subchondral insufficiency fracture of the femoral head (SIFFH) is characterized by acute onset hip pain without overt trauma. It appears as a low intensity band with bone marrow edema on T1-weighted MRI. The most common course of treatment is protected weight bearing for a period of several weeks. Total hip arthroplasty (THA) has been commonly used if the patient does not respond to the initial...
