We describe a rare case of syncope caused by complete atrioventricular (AV) block on upright posture. The index case is a 72-year-old man who presented with recurrent syncopal episodes without a significant postural drop on presentation. During his admission we demonstrated reproducible AV block on standing for 15 s.

A case of reciprocal rhythm of ventricular origin associated with atrial fibrillation and complete AV block is described. This manifestation shows that an atrial bridge is not an essential link in the reciprocal mechanism, and that the reciprocal mechanism is effected through the presence of an upper common AV nodal pathway.

A case of second degree, type II, atrioventricular block precipitated by exercise is presented. It is suggested that exercise and other vagolytic manoeuvres may be used as a means of differentiating type I from type II atrioventricular block: the conduction disturbance of type I improves with exercise, whereas that of type II deteriorates with exercise.
