Diarrhea commonly affects international travelers. Episodes are usually short-lasting, but in some patients, symptoms may persist. Clostridium difficile infection should be excluded in travelers with prolonged disease. We report what is, to our knowledge, the first reported study of patients with C. difficile-associated diarrhea after receipt of antibiotic treatment for traveler's diarrhea.

To determine whether a multihospital Clostridium difficile outbreak was associated with epidemic strains and whether use of particular fluoroquinolones was associated with increased infection rates, we cultured feces from C. difficile-infected patients. Use of fluoroquionolones with enhanced antianaerobic activity was not associated with increased infection rates.

In a prospective study of 30 patients with Clostridium difficile infection, we found that acquisition of spores on gloved hands was as likely after contact with commonly touched environmental surfaces (ie, bed rail, bedside table, telephone, call button) as after contact with commonly examined skin sites (ie, chest, abdomen, arm, hand).
