Pellagra is an exceptional skin involvement in celiac disease. Ischemic cardiomyopathy is another rare event in celiac disease. Its pathogenesis remains not yet elucidated. We report a case of a 34year old patient of presenting with pellagra and cardiomyopathy revealing a celiac disease.
