A girl had repeated episodes of widespread osteolysis, subcutaneous fat necrosis, and periarticular fat necrosis mimicking arthritis. Her symptoms persisted for 4 years, after in initial episode of severe abdominal pain, and were accompanied by only minor abdominal symptoms. Heterotopic ossification is characterised by the periarticular deposition of ectopic bone. It typically occurs after trauma, neurogenic injury, or congenital causes. Idiopathic heterotopic ossification has been rarely reported. Periarticular tissue calcifications are common in patients with chronic renal failure undergoing hemodialysis. We describe a patient with rheumatoid arthritis and widespread joint and periarticular calcinosis related to self-medication with vitamin D, which was aggravated by oral phosphate therapy prescribed for her hypercalcaemia. Hydroxyapatite was shown in the synovial fluid from affected joints. The role played by tissue injury in the pathogenesis of soft tissue calcification is discussed.
