傍前床突起部動脈瘤は現在血管内治療が第一選択である．しかしながら，クリッピング手術が必要となる症例もまれながら存在するため，その手技や経験は伝えていくべきものであると考えられる．こういった観点から，われわれが過去に同部位動脈瘤に対し反対側からクリッピングを行った3例について報告する．本法は前床突起を削除する必要がなく，上下垂体動脈の同定操作も容易であり全例通常のクリップで可能であり，術後視力視野障害の合併はなかった．本法は親血管の中枢側の確保に問題があるためその適応は非常にかぎられてはいるものの，比較的小さな傍前床突起部上下垂体動脈瘤に対して視障害回避のために有用であると考えられた．
