Extraintestinal respiratory manifestations in inflammatory bowel disease (IBD) are rare. We present a case of bronchiolitis obliterans organizing pneumonia (BOOP) in a patient with Crohn s disease, with clinical remission with no drug therapy.

症例は36歳,女性.18歳時にCrohn病を発症し,痔瘻を含む大腸病変の増悪のため28歳時に回腸人工肛門造設を施行されている.8年後,大腸病変および直腸周囲膿瘍の再燃のため大腸全摘術を施行したところ,回腸断端~結腸・直腸全域にわたって無数に重複する同時多発性のCrohn病関連癌が診断された.Crohn病の長期経過に伴い炎症性病変を母地とした癌が発生することは周知されているが,潰瘍性大腸炎の場合のように多発癌に関しては本邦ではまだ議論がなされていない.本邦におけるCrohn病関連癌の疫学が著しく欧米と異なるため,癌に対するサーベイランス方の確立途上であることも一因と思われる.疫学や治療法に対する欧米と本邦の差異を考察に加え報告する.
