We present an iatrogenic, pleuro-pericardial connection resulting from pericardiocentesis of a large, tuberculous, pericardial effusion. Recognition of this situation is paramount when one is unable to aspirate pericardial fluid after a successful, initial puncture. Such knowledge will help prevent myocardial or coronary artery injury with further attempts at aspiration.

We report the first case of foetal pericardial teratoma, treated successfully in utero by pericardio-amniotic shunting, and review the relevant literature. Foetal treatment improves survival in case of hydrops. Foetal pericardio-amniotic shunting could possibly be an alternative to serial pericardiocentesis.

We describe a case of pericardial tamponade as an initial manifestation of systemic lupus erythematosus (SLE). Although pericarditis or pericardial effusion is the common cardiac complication of SLE, tamponade is unusual. Treatment consists of pericardiocentesis, administration of high dose glucocorticoid an antimalarial drug.
