【目的】関節面が陥没した肘頭骨折に対して引き寄せ鋼線締結法（TBW）を2組用いるdouble TBW（DTBW）を行い，その治療成績を調査した．【対象】2017年1月以降Colton分類2C/2Dに対しDTBWを行った6例．【方法】陥没骨片を1組目のTBWで支持させ鋼線を前方皮質に貫いた．2組目のTBWは髄内遠位に挿入し鋼線近位端を鉤状に曲げた．関節面の矯正損失，骨癒合，肘関節可動域，Mayo Elbow Performance Score（MEPS），合併症について調査した．【結果】全例で関節面の矯正損失なく骨癒合が得られた．可動域は平均－9°～127°，MEPSは平均93.0点であった．合併症は表層感染が1例あった．鋼線のバックアウトは50％ にみられたが皮膚刺激症状は生じなかった．【考察】関節面が陥没した肘頭骨折に対するDTBWの治療成績は良好であった．
