The anatomy of the first metatarso-phalangeal joint and of dorsal dislocation of the phalanx are described. As similar lesions in the hand, closed reduction is impossible because of interposition of the volar plate. Open reduction is essential and should be performed as soon as possible after the injury.

Injuries to the tarsometatarsal joint and of the Lisfranc ligament present a challenge. They are difficult to diagnose and outcomes worsen as diagnosis is delayed. As a result, radiologists and clinicians must have a clear understanding of the relevant nomenclature, anatomy, injury mechanisms, and imaging findings.

The authors present an case of a thumb metacarpophalangeal joint dislocation which was made complicated by the interposition of the sesamoid bone which required open reduction. The intact volar plate prevented closed reduction. The anatomy and surgical management of this unusual case is described.

A surgical approach is described that has been used in nearly 200 primary cases of Dupuytren's disease treated by one surgeon. Special attention and comments are directed to the proximal interphalangeal joint, a notoriously difficult area to correct.
