A 5-year-old boy was found to have a congenital left ventricular outflow tract (LVOT) aneurysm of the intervalvular fibrosa, LVOT obstruction after repair of a perimembranous ventricular septal defect, and aortic coarctation. The patient underwent successful plication of the aneurysm, resection of the fibrous subaortic stenosis, and septal myectomy.

Surgery on inferior oblique muscle has a long history and various surgical techniques have been developed. The historical review of the surgery starting in 1840's to date is performed. The surgical effectiveness between simple myectomy and recession is still controversial.

Surgical treatment appears to be the therapy of choice in hypertrophic, obstructive cardiomyopathy, at least in patients who are in the advanced stage of the disease and refractory to medical treatment. In addition, strong evidence has been obtained that surgical treatment improved the prognosis at least in patients who required only a myectomy via a transaortic approach.
