Drug-eluting balloon (DEB) with angioplasty a paclitaxel-coated balloon catheter is an effective treatment option in patients with in-stent restenosis (ISR) after a drug-eluting stent (DES). We describe a case in which 'no-reflow' phenomenon developed after DEB angioplasty of a DES ISR lesion. Coronary flow was restored after intracoronary administration of nicorandil.
Studies were performed to determine whether an alteration in coronary vascular resistance and a reduction in the reflow phenomenon occurred in the blood-perfused, heparinized canine heart after various periods of myocaridal ischemia. Regional myocardial blood flow was measured with radioactive microspheres. Proximal left anterior descending coronary artery blood flow was measured with a periart...
The interventional cardiologist makes a provisional diagnosis of the no-reflow phenomenon in the presence of an acute reduction in coronary flow despite a widely patent epicardial vessel during percutaneous coronary intervention (PCI). Its occurrence is recognized as a column of contrast arising distal to the original target stenosis that does not rapidly clear [1–3]. The precise pathophysiolog...
After several hours of ischemia an incomplete return of blood flow has been reported in brain, kidney, skeletal muscle, and heart. The mechanisms responsible for the no-reflow phenomenon have been unclear, and perivascular edema, platelet or red cell plugs, and interstitial hemorrhage have been implicated. In the present study evidence is provided that leukocyte entrapment in capillaries might ...
The achievement of adequate and prompt myocardial reperfusion in acute myocardial infarction (AMI) is the most important determinant of recovery of ventricular function at follow-up. This explains the great attention given to the so-called no-reflow phenomenon, which is associated with extensive myocardial damage and a high prevalence of future cardiac events after AMI. The no-reflow phenomenon...
A 73 year old man was admitted with haematemesis and was found to have a pangastritis. Three days later he developed central chest pain, and ECG changes indicated an acute anterior myocardial infarction. In view of his recent haematemesis we performed emergency coronary angiography, which showed proximal occlusion of the left anterior descending (LAD) artery (TIMI 0 flow), with normal circumfle...
