BACKGROUND Little is known about surgeon agreement and accuracy using arthroscopic evaluation of anterior cruciate ligament (ACL) tunnel positioning. PURPOSE To investigate agreement on ACL tunnel position evaluated arthroscopically between operating surgeons and reviewing surgeons. We hypothesized that operating and evaluating surgeons would characterize tunnel positions significantly differ...

Methods: In a prospective, double-blind, randomized clinical study, 100 patients eligible for ACL reconstruction with a transtibial technique were stratified by surgeon and randomized to either conventional or computer-assisted surgery. Measurement of femoral and tibial tunnel placement with use of three-dimensional computed tomography (CT) was used as the primary outcome to compare conventiona...

A technique for stabilising the superior tibiofibular joint using an autogenous biceps graft passed through a tibial tunnel is described. The common peroneal nerve should be decompressed and the lateral inferior genicular artery protected. The technique proved to be safe and effective in two patients who were followed for at least two years.

Predictive ability of a positive Tinel sign over the tibial nerve in the tarsal was evaluated as a prognostic sign in determining sensory outcomes after distal tibial neurolysis in diabetics with chronic nerve compression at this location. Outcomes were evaluated with a visual analog score (VAS) for pain and measurements of the cutaneous pressure threshold/two-point discrimination. A multicente...

BACKGROUND Primary stability of the graft is essential in anterior cruciate ligament surgery. An optimal method of fixation should be easy to insert and provide great resistance against pull-out forces.A controlled laboratory study was designed to test the primary stability of ACL tendinous grafts in the tibial tunnel. The correlation between resistance to traction forces and the cross-section ...

A survey was conducted of the Herodicus Society membership to assess the current status of treating posterior cruciate ligament (PCL) injuries and compared to a similar survey conducted in 1991. The survey addressed indications, graft choice, surgical technique, graft tensioning, and graft fixation. Seventy-eight active surgeons were sent a 34-question survey pertaining to isolated PCL injuries...

BACKGROUND Recently, anatomic anterior cruciate ligament (ACL) reconstruction is emphasized to improve joint laxity and to potentially avert initiation of cartilage degeneration. There is a paucity of information on the efficacy of ACL reconstructions by currently practiced tunnel creation techniques in restoring normal joint laxity. STUDY DESIGN Controlled laboratory study. HYPOTHESIS Ante...

The physical status of an interference screw fixation in an anterior cruciate ligament (ACL) reconstructive surgery is a crucial factor of success. In this study, the insertion of the interference screw into a tibial bone tunnel, and the advancement of the screw between the tendon strands were investigated using both experimental and computer modeling. The mechanical behavior of the bone and so...

Tarsal Tunnel Syndrome (TTS) is an entrapment neuropathy of tibial nerve which is compressed in the tarsal tunnel, an anatomic structure formed between bones (medial wall of calcaneus, talus and distal tibia) and flexor retinaculum. The cases are usually idiopathic whereas occasionally TTS occurs secondary to space occupying lesions such as ganglia, tumors, bony protrusions [1-8]. Tarsal coalit...

Anterior cruciate ligament (ACL) rupture predisposes to altered kinematics and possible knee joint degeneration. Graft fiber maturation and ligamentization may eliminate this risk during ACL reconstruction procedures. ACL remnant-sparing techniques support the theory that the preserved tissue enhances revascularization, preserves the mechanoreceptors, and leads to anatomic remodeling. The purpo...
