A patient with secondary hyperparathyroidism due to chronic renal failure developed epidermal and follicular calcification without concomitant dermal calcification in the lesions of toxic epidermal necrolysis. We propose that the combination of keratinocyte necrosis and an elevated serum calcium-phosphorus product resulted in epidermal calciphylaxis.
Abstract Background: Toxic epidermal necrolysis (TEN) and Stevens Johnson syndrome (SJS) are diseases that may cause serious complications. This study aims to describe the cause, clinical course and complications in patients admitted with TEN and SJS in two university hospitals in Tehran. Materials and methods: Patients admitted with TEN or SJS in Shohada and Loghman hospitals between 2000 an...
Cytokines are important regulators of immune and inflammatory reactions in the skin, and may contribute to inflammatory blister induction. We examined the profiles of interleukin-6 (IL-6) and tumour necrosis factor-alpha (TNF-alpha) in fluid of spontaneous blisters in the immune-based inflammatory disorders bullous pemphigoid (8 patients), allergic contact dermatitis (5 patients) and toxic epid...
BACKGROUND Adverse drug reactions are harmful and involuntary responses to drugs that occur at doses normally used for a given condition. Among them are Stevens-Johnson syndrome and toxic epidermal necrolysis, both rare and potentially fatal conditions. OBJECTIVES To analyze the epidemiological and clinical characteristics related to patients diagnosed with Stevens-Johnson syndrome and toxic ...
Cutaneous adverse reactions are reported for many therapeutic agents and, in general, are observed in between 0% and 8% of treated patients depending on the drug. Antiosteoporotic agents are considered to be safe in terms of cutaneous effects, however there have been a number of case reports of cutaneous adverse reactions which warrant consideration. This was the subject of a working group meet...
Toxic epidermal necrolysis is a rare, severe cutaneous reaction, mostly caused by drugs. It affects the skin and mucous membranes, with involvement of more than 30% of body surface. We describe the case of a young woman, previously healthy, who developed skin detachment of more than 90% of the body surface 15 days after being administered lansoprazole for peptic disease. The treatment consisted...
Three cases are described in which Stevens-Johnson syndrome progressed in the course of a few days to toxic epidermal necrolysis. Trimethoprim-sulfamethoxazole, allopurinol in combination with hydrochlorothiazide, phenytoin and possibly ampicillin were implicated in the causation of the disease.
Background: Toxic epidermal necrolysis (TEN) is a severe life-threatening condition associatedwith a low incidence but high mortality rate ranging from 25-50%. TEN is characterized by itswidespread skin necrosis with frequent mucous membrane involvement. The pathophysiology ofTEN has yet to be fully understood, however the current understanding involves a limited scope.Objec...
A 26-year-old male was referred from elsewhere for the evaluation of oral ulcers, joint pains, renal failure and had already received 2 weeks of oral prednisone. His immunological evaluation showed positive anti nuclear and anti-double-stranded antibodies. Renal histology at our hospital was suggestive of diffuse proliferative lupus glomerulonephritis and the patient was put on oral prednisone ...
