症例は84歳，男性．5時間続く腹痛と嘔吐を主訴に，当院へ救急搬送された．腹部造影CTでclosed loopを認め，絞扼性腸閉塞と診断し緊急手術を施行した．開腹すると小腸とS状結腸が結節を形成するileosigmoid knotを呈し，特に小腸は広範囲に渡ってS状結腸に巻絡し壊死していた．小腸一括切除は短腸症候群が懸念されたため，絞扼を解除し，壊死範囲を確認して速やかに小腸とS状結腸をそれぞれ切除吻合した．残存小腸は130cmと短くなったが，短腸症候群をきたすことなく，術後13日目に軽快退院した．壊死を伴うileosigmoid knotは広範な小腸切除を要することが多く，絞扼を解除しない小腸一括切除は過剰切除によって短腸症候群をきたし，術後のQOLを著しく損なうリスクがある．残存小腸が短くなることが予想される症例では，全身状態が許容されるなら絞扼を解除し，切除範囲を最小限に留めるこ...
