2020年，22カ国32名の専門医からなるInternational Expert Panelにより脂肪肝の新概念metabolic dysfunction-associated fatty liver disease（MAFLD）が提唱された．MAFLDは脂肪肝に「肥満」，「2型糖尿病」，「2種類以上の代謝異常」のいずれかが併存している疾患概念である．代謝異常は脂肪肝の病期進展にかかわる重要な危険因子であり，その危険因子を組み入れ基準としたMAFLDはハイリスク脂肪肝患者の同定に有用と考えられる．本稿ではMAFLDの特徴を論述するとともに，MAFLDと肝関連イベント，および動脈硬化性心血管疾患や肝外悪性腫瘍などの肝外イベントとの関連について論じる．また，MAFLDが疾患啓発や脂肪肝の治療薬開発におよぼす影響について述べるとともに，疾患名に関する今後の動向についても紹介する．
