Isolated unicompartmental osteoarthritis in the young patient is a difficult problem to treat; they may be too young to consider total knee arthroplasty due to difficulties with inevitable future revision. Unicompartmental knee arthroplasty is one possible solution as it is perceived by some as being a smaller surgical insult than total knee arthroplasty, with easier revision to total knee arth...

The issue of athletic participation after hip and knee arthroplasty has become more relevant in recent years, with an increase in the number of young and active patients receiving joint replacements. This article reviews patient-, surgery-, implant-, and sports-related factors, and discusses currently available guidelines that should be considered by the physician when counseling patients regar...

PURPOSE Bony resection of the proximal medial tibia, an alternative technique for soft tissue balancing in total knee arthroplasty (TKA), was compared to the conventional medial soft tissue release technique. MATERIALS AND METHODS From June 2005 to June 2007, we performed 40 TKA in 27 patients with ≥10° tibio-femoral varus deformity. The conventional, medial soft tissue release technique was ...

Introduction: Equality between the extension gap (EG) and the flexion gap (FG) is one of the most important factors for good clinical results in total knee arthroplasty (TKA). The gap situation during surgery has a wide variation. The EG needs to be widened in patients with varus knees that have a small EG. For this purpose, posterior soft tissue release is necessary to avoid additional resecti...

INTRODUCTION The total hip arthroplasty (THA) as part of acute fracture management is used for acetabular fractures in elderly patients. Our objective was to assess the stability of osteosynthesis performed using 2 different techniques in combination with THA in an experimental model. MATERIALS AND METHODS We conducted 20 experiments using the left-side hemipelves composite bone models. There...

Excision arthroplasty of the shoulder is a rarely performed procedure used in cases where replacement of a failed arthroplasty is not feasible. We report four cases of excision arthroplasty following a shoulder replacement, one performed for infection and three for instability. Excision arthroplasty resulted in very poor function with pain improvement in only two cases.

In contrast to cervical and lumbar fusion procedures, the principal aim of disk arthroplasty is to recapitulate the normal kinematics and biomechanics of the spinal segment affected. Following decompression of the neural elements, disk arthroplasty allows restoration of disk height and maintenance of spinal alignment. Based on clinical observations and biomechanical testing, the anticipated adv...

One component of conventional total knee arthroplasty is removal of the anterior cruciate ligament, and the knee after total knee arthroplasty has been said to be a knee with anterior cruciate ligament dysfunction. Bicruciate stabilized total knee arthroplasty is believed to reproduce anterior cruciate ligament function in the implant and provide anterior stability. Conventional total knee arth...

BACKGROUND The optimal surgical management of displaced femoral neck fractures in the elderly remains controversial. Treatment alternatives include arthroplasty and internal fixation. Options for arthroplasty include total hip arthroplasty and hemiarthroplasty, whereas options for internal fixation include multiple screws and sliding hip screws. We sought to compare arthroplasty and internal fi...
