OBJECTIVE Lung resection for complex aspergilloma (CA) carries high morbidity and mortality and remains controversial in high-risk patients. Cavernostomy followed by muscle-flap plombage has been recommended for patients considered unfit for resection, but subsequent muscle-flap atrophy may be a main cause of failure. We reviewed the place of a limited thoracoplasty in association with that pro...

OBJECTIVE Donor site hernias are a rare but well recognised complication of deep inferior epigastric perforator (DIEP) flap breast reconstruction but there are no reported cases of epigastric hernias after such surgery. We report three patients who developed symptomatic epigastric hernias within 2-8 months after discharge from follow-up. PATIENTS AND METHODS Patients who were referred to the ...

Repeated soft tissue injuries around the knee represent challenging clinical situations where functional prognosis of the joint is often poor, especially in the presence of total knee arthroplasty (TKA). When gastrocnemius muscle flap techniques have already been used and failed, therapeutic solutions become rare. The authors suggest a regional solution to salvage these cases by the transfer of...

BACKGROUND Mastectomies are one of the most common surgical procedures in women of the developed world. The gracilis myocutaneous flap is favoured by many reconstructive surgeons due to the donor site profile and speed of dissection. The distal component of the longitudinal skin paddle of the gracilis myocutaneous flap is unreliable. This study quantifies the fasciocutaneous vascular territorie...

BACKGROUND surgical excision remains the best tool for management of skin tumors affecting nasal skin, however many surgical techniques have been used for reconstruction of the nasal defects caused by excisional surgery. The aim of this work is the evaluation of the feasibility and outcome of levator anguli oris muscle based flaps. METHODS Ninety patients of malignant nasal skin tumours were ...

The extensor digitorum brevis muscle flap is reliable, safe, and can be used either as a pedicle or as a free flap with minimal donor site morbidity. To increase the existing knowledge of this flap and to establish further anatomic basis for the design and elevation of the extensor digitorum brevis flap, 26 specimens from 13 fresh cadavers were dissected under 3.5x loupes. The lateral tarsal ar...

DESCRIPTION A 56-year-old woman with previous breast cancer underwent a muscle-sparing transverse rectus abdominis myocutaneous flap to her left breast. She developed marked flap swelling postoperatively and episodes of pyrexia. A breast wound swab grew Escherichia coli and Acinetobacter baumannii. This strain of Acinetobacter was multidrug resistant, including resistance to meropenem.

We used a free tarsal-conjunctival grate from the upper eyelid and a nasal-based pedicle skin-muscle flap for reconstruction of the medial aspect of the lower eyelid. This had the advantage of confining the reconstruction to the local area and eliminated the need for a temporal flap or an eyelid sharing procedure that necessitates occluding the fissure for several weeks.

The free muscle-sparing transverse rectus abdominis myocutaneous (MS-TRAM) and deep inferior epigastric perforator (DIEP) flaps involve transferring skin and subcutaneous tissue from the lower abdominal area and have many features that make them well suited for breast reconstruction. The robust blood supply of the free flap reduces the risk of fat necrosis and also enables aggressive shaping of...

Traumatic injuries to the distal forearm present a challenging problem for soft tissue coverage. There is a need for a local flap to provide a vascularized soft tissue bed. This article demonstrates the anatomic basis that enables the pronator quadratus muscle potentially to fulfill these needs.
