症例は40歳代,男性.健康診断の胸部X線写真で異常陰影を指摘され当院を受診した.胸部CTで前縦隔に長径10 cm大の分葉状の腫瘤を認めた.前縦隔腫瘍として完全切除を目的とした手術を行った.腫瘤は一部心膜と強固に癒着しており,心膜を切開し心囊内を確認したところ,肉眼的に腫瘍は心膜腔内に向かって鶏冠状に突出しており,心膜浸潤を疑う所見であり,心膜を合併切除した.組織学的所見と免疫染色の結果を総合し孤立性線維性腫瘍Solitary Fibrous Tumor(SFT)と判断された.SFTは画像上境界明瞭な腫瘤として描出されることが多く,肉眼的にも表面平滑で境界明瞭な腫瘤として認めることが多い.本症例のように心膜腔内に向かって鶏冠状に突出したという報告は検索の範囲では認めず,多彩な臨床像をとるSFTの一所見として貴重な一例と考え報告する.
