Regression of Urrets-Zavalia Syndrome After Deep Lamellar Keratoplasty for Keratoconus: A Case Study
We report a case of pharmacologic regression of Urrets-Zavalia syndrome, following deep lamellar keratoplasty for severe keratoconus. Sympatholytic and parasympathomimetic drops were administered and a progressive reduction of mydriasis and restoring of pupillary kinetics were observed. Four years after surgery, it was resulting in only slight residual anisocoria.
Two cases of anisocoria that occurred just after the induction of general anesthesia for living donor liver transplantation are reported. Space-occupying lesions were not observed in brain computed tomography. Mydriasis was temporary in both cases, suggesting that the anisocoria was most likely related to Adie syndrome.
Local instillation of thymoxamine eye drops (0-1%) completely reversed the mydriasis produced by ephedrine (5%) but not that produced by ephedrine (5%) together with homatropine (0-5%). Small but significant changes in accomodation were found with ephedrine and with thymoxamine, as well as the larger expected changes with homatropine.
