症例は46歳，女性．2カ月前にDouglas窩膿瘍に対して産婦人科に入院歴を有する．経過から，虫垂炎に伴う腹腔内膿瘍の疑いで，精査加療目的に当科へ紹介された．造影CTでは虫垂根部に造影効果のある2.0×1.5cmの結節と，盲腸周囲のリンパ節腫大を認めた．下部消化管内視鏡検査では虫垂開口部周囲は発赤調で腫大しており，白色膿汁の付着を認めた．FDG-PET検査ではCTでみられた結節に一致してFDGの集積を認めた．生検で悪性所見は得られなかったが，虫垂癌を否定できず腹腔鏡下回盲部切除術を施行した．病理検査では黄色肉芽腫性虫垂炎の診断であった．黄色肉芽腫性虫垂炎は稀な疾患であり，虫垂癌との鑑別は困難である．術式については一定の見解を得られておらず，患者の全身状態，意向などを考慮し慎重に選択する必要がある．
