Spinal infarction is a rare and devastating complication of open-heart surgery, especially in the context of perioperative hemodynamic instability in patients requiring high dose of inotropes and vasoconstrictors. Our report highlights that spinal infarction can occur in such circumstances following a valve replacement surgery.

•Pericardial effusion may be present early in the disease process in patients with ovarian cancer.•Even a small pericardial effusion may become clinically apparent in the setting of concomitant pleural effusion(s).•Cardiac tamponade should be considered in the differential diagnosis of postoperative hemodynamic instability to facilitate timely intervention.

Little exists on treatment for SAAs rupture which may require a fast bleeding control because of the hemodynamic instability and a large perisplenic/peritoneal hematoma.
