A patient with mitral stenosis and systemic thromboemboli is presented. Two-dimensional echocardiography demonstrated a spherical thrombus floating freely within the left atrium which was corroborated by postmortem examination. Although they can be discovered by a transthoracic echocardiography, they may need histological examination to differentiate them from other cardiac masses.
