Variants of exstrophy are rare anomalies seen in the spectrum bladder exstrophy-epispadias complex. A seven day old male neonate was admitted our department with a variant duplicate communicating fistula. Patient presented patch exstrophic just below low set umbilicus normal sized phallus and normally positioned urethral meatus. The baby managed successfully surgery is under regular follow up.
