肺内型気管支原性囊胞は先天性囊胞性肺疾患の一型であり，幼児期の胸腔鏡下手術が推奨されているが，実際に経験することは稀である．今回，我々は，出生前診断された後縦隔囊胞性病変に対して，1歳5ヵ月時に外科的介入を行った．8K胸腔鏡を用いて小児外科・呼吸器外科合同で手術を施行し，右下葉S8から隆起する囊胞性病変を認めたことから，肺内型気管支原性囊胞と診断した．下肺静脈との癒着のため最終的にはS8区域切除でなく右下葉切除を要したが，高精細映像を70インチの大画面で共有することにより，肺動脈・気管支の剥離を各区域支レベルで安全に進めることができ，胸腔鏡補助下手術を完遂し得た．8K胸腔鏡により得られる拡大視効果やズームアップ機能は，狭い小児の胸腔で作業空間を確保し，精度の高い低侵襲手術を行う上で非常に有用であると考えられた．
