目的：回結腸動脈，右結腸動脈（RCA）が支配血管の上行結腸癌（以下，近位上行結腸癌）に対する腹腔鏡下結腸切除術（以下，腹腔鏡下右結腸切除術）の短期・長期成績にRCAの有無が与える影響を明らかにする．方法：腹腔鏡下右結腸切除術D3郭清を施行した近位上行結腸癌96例をRCAの有無に従い2群に分け，臨床病理学的因子，短期・長期成績を検討した．結果：RCAあり（RCA群）は51例，RCAなし（non-RCA群）は46例で，臨床病理学的因子に差はなかった．RCA群ではnon-RCA群よりも手術時間が長く（p＝0.047），リンパ節郭清個数が多く（p＝0.003），特に主リンパ節郭清個数（p＝0.044）が多かった．合併症率や長期成績に差はなかった．結語：近位上行結腸癌に対する腹腔鏡下右結腸切除術においてRCAの有無による短期・長期成績の差はないと考えられた．
