臨床症状の有無に関わらず，咬合に問題を抱えている患者は多い．そのような患者に対して良好な口腔内環境を獲得し永続させるためには，不良な咬合関係を是正し，顎口腔系（顎関節，口腔周囲筋，歯・歯周組織）の調和を図る必要がある．本稿では，患者の病態を「歯列不正」，「歯周病」，「多数歯欠損」の三つに分類した．それぞれが持つ特徴と治療のポイントがどこにあるのかを症例を提示して解説する．「歯列不正」の解消には矯正治療が有効であり，「歯周病」の進行した症例では，効率的な連結固定が有効である．「多数歯欠損」における咬合再構成はインプラントの利用が効果的であり，治療をシンプルにする．咬合再構成は患者個々の病態に応じた治療法を的確に適応して，長期的に安定した咬合状態を維持できるように再構成する必要がある．
