Seven patients with subdural empyema were initially treated by antibiotics without surgery. Six have recovered without sequelae. One required delayed surgery and has recovered with epilepsy. The authors emphasise the use of CT for the diagnosis and follow-up of subdural empyema, the principles and modalities of non-surgical treatment, and the good results, especially for late morbidity.

− Subdural electrodes are useful in selected patients with cortical dysplasia for planning resective epilepsy surgery. Their potential risks have to be balanced against the diagnostic yield and expected benefit of epilepsy surgery for the patient. Recent advances in imaging techniques now allow more accurate placement of the subdural electrodes and also limit the number required.

The Mount Fuji sign is seen in bilateral subdural hypertensive pneumocephalus. These air collections cause compression of the frontal lobes, which take a shape similar to the silhouette of Mount Fuji. It is most commonly seen after surgical decompression of a chronic subdural hematoma. However, it may also be observed following head trauma, otogenic infections, nitrous oxide anesthesia and diving.

The report on subdural empyema by EK Ranjini and colleagues alerts us to the severity of pneumococcal meningitis in young infants(1). There are several important lessons to be learned from this re...
