The aetiology, incidence and management of type II endoleaks in standard infrarenal endovascular aortic aneurysm repair is well described. Far less data is available for thoracic stent grafting. We present a rare and interesting case of a type II endoleak post thoracic aortic stenting and highlight the aberrant anatomy that can cause this phenomenon in such cases.
BACKGROUND In a population-based study in Tromsø, Norway, the authors assessed whether an abdominal aortic aneurysm (AAA) or the maximal infrarenal aortic diameter in a non-aneurismal aorta influence total and cardiovascular disease (CVD) mortality. METHODS A total of 6640 men and women, aged 25-84 years, were included in a 10-year mortality follow-up: 345 subjects with a diagnosed AAA and 62...
Aneurysmal arterial disease is a vascular degenerative condition that is distinct from atherosclerotic and other occlusive arterial diseases. There is regionalization of the predisposition to aneurysm formation within the vascular tree, and the pathological process varies with location. Infrarenal abdominal aortic aneurysm (AAA) is the most common manifestation of aneurysmal disease, and smokin...
Background Abdominal aortic aneurysms (AAA) are a major cause of morbidity and mortality in the US. The most common location of AAA is the infrarenal abdominal aorta where oscillatory flow is present over the cardiac cycle, figure 1. Oscillatory flow is known to initiate an inflammatory response in endothelial cells. Patients with lower extremity traumatic amputations have a 5-fold increase in ...
because of significant reduction of rupture rate and increase in the patient’s life expectancy, elective surgical treatment in patients with abdominal aortic aneurysm has been widely accepted. the present article aims at assessing the postoperative complications and mortality rate in patients who had been submitted to elective surgery for abdominal aortic aneurysms. this is a retrospective stud...
Endovascular repair (EVAR) of abdominal aortic aneurysms (AAA) is an established alternative option to conventional surgery for AAA, provided optimal anatomical morphology of the aneurysm sac, neck and outflow exists. In most documented series of EVAR, type-II endoleak occurrence is a universal procedural drawback. This is referred to as the Achilles heel of EVAR. This morphological study, addr...
Bladder and rectal incontinence without paraplegia or paraparesis after endovascular aneurysm repair
Spinal cord ischemia is a well-known potential complication of endovascular aneurysm repair (EVAR), and it is usually manifested by paraplegia or paraparesis. We describe a case in which spinal cord ischemia after EVAR presented by isolated bladder and rectal incontinence without other neurological deficits. A 63-year-old woman presented with intermittent claudication secondary to an infrarenal...
Abdominal aortic aneurysm (AAA) is a significant source of morbidity and ranked by the Centers for Disease Control and Prevention as the 15th leading cause of death among adults aged 60 to 64 years. Size confers the largest risk factor for aneurysm rupture, with aneurysms >6 cm having an annual rupture risk of 14.1%. We present the case of a 60-year-old man found on ultrasound imaging at a heal...
Atherosclerosis is a chronic condition characterised by the formation of lipid-rich plaques within the walls of medium and large arteries (Ross, 1993; Lusis, 2000) and underlies many forms of vascular disease, including abdominal aortic aneurysm (AAA). The development of all vascular disease phenotypes is dependent on multiple genetic and environmental determinants (Lusis, 2000) though the rela...
Abdominal aortic aneurysm is included in the differential diagnosis of lower back pain. Although rare, this important disease can cause potentially lethal complications. In this case, expanding abdominal aortic aneurysm coexisted with intervertebral disc extrusion. The diagnosis of abdominal aortic aneurysm was delayed, putting the patient at risk of aneurysmal rupture. In the management of pat...
