An acute confusional state, which developed in a patient with polyarteritis nodosa (PAN), proved to be secondary to inappropriate secretion of antidiuretic hormone and consequent hyponatraemia. This is a very unusual complication of PAN and may well reflect a direct stimulation of the supraoptic nuclei owing to cerebral vasculitis.
Six cases of tuberculous meningitis, all occurring in patients over the age of 65, are described. All patients presented with an acute illness, primarily with a confusional state. Headache was a symptom in only two patients and the cases were not confined to ethnic minority groups. The mortality was 50%.
Electroencephalograms of 95 patients with an acute confusional state, referred over three years, were studied to determine their usefulness. The majority were abnormal, and those from patients with intracranial pathology often showed asymmetry of delta activity and paroxysmal discharges.
