Lopes et al1 recently published in The New England Journal of Medicine the results of a comparative study between open and endoscopic vein harvesting (EVH) in coronary artery bypass surgery. This study did not pass unnoticed. An impressive record of 1000 patients per arm, 1800 angiographies at 12 to 18 months, and a clinical 3-year follow-up were analyzed. Surprisingly, the results of the 4000 ...

Endoscopic vein harvesting (EVH) is becoming common for the patients undergoing coronary artery bypass grafting. Using carbon dioxide insufflations during the vein harvest can produce rare but catastrophic CO(2) embolism. We report a case of massive right atrial CO(2) embolism due to femoral vein injury which occurred during the performance of a routine EVH procedure.

BACKGROUND AND OBJECTIVES The greater saphenous vein has been used in coronary artery bypass grafting (CABG) for more than 50 years. Endoscopic vein harvesting has greatly reduced the morbidity associated with obtaining the vein, but the quality of the vein could not be assessed before its was exposed surgically or after the endoscopic procedure had been performed. This study was conducted to e...

The endoscopy-assisted technique has been demonstrated in harvesting muscle flaps; however, for pedicled freestyle fasciocutaneous flaps, few studies have applied this technique. We present a surgical procedure utilizing endoscopic-assisted method to identify the perforators of pedicled freestyle fasciocutaneous flaps for the reconstruction of soft tissue defects. From August to December 2012, ...

Harvesting of the saphenous vein is a routine procedure in coronary and peripheral vascular surgery. It is usually performed using a continuous long skin incision. Minor complications are reported in up to 24% (hematoma, wound dehiscence, infection, pain) and major problems necessitating surgical interventions (bleeding, abscess) in less than 1%. These complications lead to a prolonged hospital...

BACKGROUND Endoscopic vein harvesting (EVH) is the standard of care for coronary artery bypass grafting (CABG) in the United States, but recent comparisons with open harvesting suggest that conduit quality and outcomes may be compromised in EVH. To test the hypothesis that problems with EVH may relate to its learning curve and conduit quality, we analyzed the quality and early function of condu...
