Antibiotic impregnated articulating spacers are used in two-stage revision total knee arthroplasty to deliver local antibiotic therapy while preserving function. We have observed infection control in greater than 95% of cases with functional outcomes approaching those seen in revision for aseptic loosening. Higher failure has been observed with methicillin resistant organisms.
BACKGROUND I conducted a prospective study to assess the effectiveness of an absorbable calcium hydroxyapatite (Hac) layer mixed with vancomycin applied to the articular surface of prosthetic implants in preventing deep infections after noncemented total knee arthroplasty (TKA). This severe complication of TKA occurs in 2%-7% of cases. METHODS In all, 135 consecutive noncemented TKAs were per...
OBJECTIVES Negative pressure wound therapy (NPWT) has been used to help manage open wounds. Surgeons also often use local antibiotic depot as adjunctive therapy in an effort to reduce infection rates. These 2 techniques have been reported to be used in conjunction, but there are little data to support this practice. We sought to compare the contamination levels of wounds treated with the common...
introduction: antibiotic therapy before or after orthognathic surgery is commonly recommended by surgeons to minimize the risk of wound infection. this article evaluates the value of prophylactic antibiotic therapy in order to diminish the incidence of postoperative wound infection after orthognathic surgery. materials and methods: fifty candidates for bimaxillary orthognathic surgery were di...
Immediate internal fixation of severe open tibial fractures usually is contraindicated due to the high risk of infection. The objective of this study was to evaluate the efficacy of local antibiotic therapy with biodegradable poly-(DL-lactide-co-glycolide) cefazolin-loaded microspheres for the prevention of infection in experimental open fractures stabilized with internal fixation. Rabbits with...
Where local or international guidelines are available they should be used to guide therapy. Peritoneal dialysate effluent should be collected and processed in an appropriate manner to ensure culture-negative episodes account for < 20% of all PD-associated peritonitis. While there is no good evidence to support specific antibiotic choice, empiric intraperitoneal therapy should consider local mic...
OBJECTIVE 1/ To evaluate if empirical antibiotic prescription on admission to our intensive care unit (ICU) respects the local recommendations for antibiotic prescription and to identify predictors of nonadherence to these guidelines. 2/ To assess whether nonadherence to the guidelines is associated with increased in-hospital mortality due to the initial infection. MATERIALS AND METHODS This ...
Principles of judicious antibiotic prescribing for upper respiratory tract infections in pediatrics.
Most upper respiratory tract infections are caused by viruses and require no antibiotics. This clinical report focuses on antibiotic prescribing strategies for bacterial upper respiratory tract infections, including acute otitis media, acute bacterial sinusitis, and streptococcal pharyngitis. The principles for judicious antibiotic prescribing that are outlined focus on applying stringent diagn...
