特発性肺線維症(IPF)を代表疾患とする間質性肺炎は,経過中に急速に呼吸不全が悪化することがあり急性増悪(AE)と呼ばれている.手術侵襲や全身麻酔はAEの誘因となると考えられており,周術期には特別な管理が必要である.また,日本人は特にAEを発症しやすいとされている.本邦の多施設共同研究では,間質性肺炎合併肺がんの術後に9.3%の患者がAEを発症し,そのうち43.9%が死亡している.発症のリスク因子として肺活量の低下,CT画像上のUIPパターンに特に留意する必要がある.間質性肺炎合併患者に対する周術期の酸素化については,麻酔科医が危険だと感じない最低濃度の酸素を投与することが望ましいと考えられる.
