• In patients with established evidence-based indications for inhibition of the renin–angiotensin system, for example congestive cardiac failure or chronic kidney disease with proteinuria, this treatment should not generally be withheld on the basis of renovascular disease. • When an angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) are used in patients with re...
The effects of calcium supplementation on blood pressure and its mechanisms were investigated in two-kidney, one clip renovascular hypertensive rats. Two series of experiments were performed: one was begun just after renal artery constriction, the other after the onset of hypertension. Calcium supplementation significantly attenuated the development of hypertension (systolic blood pressure: 183...
OBJECTIVE Changes in renal hemodynamics occur in patients with severe COPD, especially during an acute exacerbation. Renal hemodynamics are affected by changes in oxygen and carbon dioxide levels, but these changes have not been well defined, particularly in the acute clinical situation. We wished to determine whether oxygen or carbon dioxide levels have the predominant effect on renal hemodyna...
BACKGROUND Increased sympathetic nerve activity may contribute to the progression of renovascular hypertension. Because previous results have been inconclusive, we investigated whether renovascular hypertensives show increased total and regional sympathetic nerve activity. METHODS AND RESULTS Sixty-five patients underwent renal angiography and measurements of plasma renin activity and angiote...
The aim of the present cross-sectional study was to investigate whether activation of the renin-angiotensin system in renovascular disease affects the cytochrome P450 omega/omega-1 hydroxylase (20-hydroxyeicosatetraenoic acid [20-HETE]) and epoxygenase (epoxyeicosatrienoic acids [EETs]) pathways of arachidonic acid metabolism in vivo, each of which interacts with angiotensin II. Plasma concentr...
To the Editor: We read with interest the article, “Losartan Reduces Oxidative Stress Within the Rostral Ventrolateral Medulla of Rats With Renovascular Hypertension,”1 by Nishi et al., which reported that peripheral angiotensin II type 1 (AT1) receptor blockade using losartan reduces oxidative stress within the rostral ventrolateral medulla (RVLM) in renovascular hypertension. Renovascular hype...
The aim of the present cross-sectional study was to investigate whether activation of the renin-angiotensin system in renovascular disease affects the cytochrome P450 / -1 hydroxylase (20-hydroxyeicosatetraenoic acid [20-HETE]) and epoxygenase (epoxyeicosatrienoic acids [EETs]) pathways of arachidonic acid metabolism in vivo, each of which interacts with angiotensin II. Plasma concentration and...
A 24-year-old man whose clinical features were suggestive of renovascular hypertension was referred for captopril renal scintigraphy. Captopril renal scintigraphy was positive for renovascular hypertension only when the left kidney was analyzed in 2 separate regions. Angiography confirmed severe stenosis (90%) in the upper branch of the left renal artery.
Five children with renovascular hypertension were treated with percutaneous transluminal angioplasty, and followed for 12 to 32 months. Blood pressure fell to normal in four patients and improved in one. Angiographic and endocrinological improvements were associated with a fall in blood pressure and no complications were observed. Angioplasty is considered a safe, effective treatment for childr...
