For the anterior segment surgeon, the implantation of Boston type 1 keratoprosthesis is a multistep process that begins with careful patient selection. Success depends on thorough preoperative evaluation, detailed surgical planning, and frequent postoperative follow-up. New practice patterns have emerged for each of these phases as the international experience with keratoprosthesis grows. This ...
Osteo-odonto keratoprosthesis (OOKP) is a multi-staged procedure for the treatment of blindness due to bilateral severe endstage corneal disease. Developed by Strampelli over fifty years ago and modified thereafter by Falcinelli, the OOKP is by far the only keratoprosthesis that has been retained in eyes for over twenty to thirty years. This procedure can be performed in eyes with bone dry and ...
PURPOSE To report indications, practices, complications, and outcomes from the first multicenter study on the Boston Type 1 keratoprosthesis. DESIGN Prospective, noncomparative, interventional case series. PARTICIPANTS We analyzed 141 Boston Type 1 keratoprosthesis surgical procedures, from 17 surgical sites, done from January 2003 through September 2005 in 136 eyes of 133 patients. METHO...
PURPOSE To describe the incidence, timing, visual significance, and management of posterior segment complications after Boston type I keratoprosthesis implantation. METHODS A retrospective chart review was conducted of all consecutive Boston type I keratoprostheses implanted by a single surgeon at a single institution over a 6-year period. Preoperative demographics, postoperative complication...
The type 1 Boston Keratoprosthesis preparation requires a 3-mm central punch and an 8.5 mm or larger punch in the carrier tissue. These punches are ideally concentric, but we have found difficulty in achieving concentric punches when the larger punch is performed first. We present a modification in the preparation procedure to help minimize centration error.
BACKGROUND Artificial cornea transplantation, keratoprosthesis, improves vision for patients at high risk of failure with human cadaveric cornea. However, post-operative infection can cause visual loss and implant extrusion in 3.2-17% of eyes. Long-term vancomycin drops are recommended following keratoprosthesis to prevent bacterial keratitis. Evidence, though, in support of this practice is po...
OBJECTIVE To evaluate the use and complications of a penetrating keratoprosthesis implantation in the management of corneal opacification in dogs. METHODS A retrospective clinical study describes the indications for the surgical technique utilized and the outcomes of this procedure in 20 eyes of 19 dogs with blindness of corneal origin. A successful surgical outcome was defined as a clear ker...
Purpose. To evaluate longitudinal functional and anatomical results after combined pars plana vitrectomy (PPV) and penetrating keratoplasty (PKP) using a wide-field Landers intraoperative temporary keratoprosthesis (TKP) in patients with vitreoretinal pathology and corneal opacity due to severe ocular trauma. Material and Methods. Medical records of 12 patients who had undergone PPV/PKP/KP due ...
