症例は76歳，女性．腹部エコーで右上腹部腫瘤を指摘された．CTで右上腹部に14cm大の腫瘤性病変を認め，右腎や肝臓を著明に圧排し辺縁に右副腎を認めた．内部は斑状に低吸収を呈し，造影にて辺縁に濃染を認めた．MRIではT2強調像で辺縁が低信号，内部は高信号と低信号が混在していた．副腎に発生した後腹膜chronic expanding hematoma（以下CEH）を疑った．5カ月後のCTで増大傾向を認めたため手術を施行した．周囲組織との癒着は剥離可能で，被膜を損傷することなく摘出した．標本は線維性被膜を伴う20cm大の巨大な嚢胞で，内腔に新旧混在する血腫と壊死組織，嚢胞壁に副腎を認めた．右副腎に発生した後腹膜CEHと診断した．CEHは手術や外傷を契機に発生し緩徐に増大する血腫であり，副腎原発はまれである．非常に巨大な腫瘤であったが，安全に被膜を含めた完全切除が可能であった．
