OBJECTIVE Minimal access cardiac valve surgery is increasingly utilized. We report our 11-year experience with minimally invasive aortic valve surgery. METHODS From 07/96 to 12/06, 1005 patients underwent minimally invasive aortic valve surgery. Early and late outcomes were analyzed. RESULTS Median patient age was 68 years (range: 24-95), 179 patients (18%) were 80 years or older, 130 patie...

Calcific aortic valve stenosis is the most common valvular heart disease in developed countries. Without surgery, the prognosis is extremely dismal. Therefore there is general agreement that surgical aortic valve replacement should be offered to patients with symptomatic severe aortic valve stenosis. However, surgery is denied to approx. 30-40% of elderly patients with severe, symptomatic aorti...

introduction cerebral haemorrhage could complicate post operative course after cardiac surgery, especially after multiple mechanical valves replacement needing for anticoagulation. the interruption of vitamin k antagonists in this cases, to avoid the enhancement of the cerebral haemorrhage, can be performed with reasonable safety. case presentation we report the case of a patient who underwent ...

The timing of prophylactic ascending aortic aneurysm surgery in the setting of bicuspid aortic valve disease is complex, with multiple factors influencing the decision. The 2014 ACC/AHA Valve guidelines recommend prophylactic replacement of the aortic root and/or ascending aorta once the aortic diameter exceeds 5.5 cm. This aortic size threshold for surgery is at a larger diameter than had been...

The echocardiographic features of a patient with severe aortic regurgitation due to a "floppy" aortic valve are presented. The salient abnormality observed was marked fluttering of the aortic valve cusps in diastole. Fluttering of the anterior leaflet of the mitral valve and left ventricular enlargement were noted. The diagnosis of a "floppy" aortic valve was substantiated at surgery.

BACKGROUND In patients with severe aortic stenosis at increased risk for surgery, self-expanding transcatheter aortic valve replacement (TAVR) is associated with improved 2-year survival compared with surgery. OBJECTIVES This study sought to determine whether this clinical benefit was sustained over time. METHODS Patients with severe aortic stenosis deemed at increased risk for surgery by a...

background: the evaluation of prosthetic valves is very difficult with two-dimensional transthoracic echocardiography alone. doppler and color flow imaging as well as transesophageal echocardiography are more reliable to detect prosthetic valve dysfunction. however, doppler study sometimes tends to be misleading due to the load-depending characteristics of peak and mean pressure gradients. the ...

Aortic stenosis is the most common native valve disease, affecting up to 5% of the elderly population. Surgical aortic valve replacement reduces symptoms and improves survival, and is the definitive therapy in patients with symptomatic severe aortic stenosis. However, despite the good results of classic surgery, risk is markedly increased in elderly patients with co-morbidities. Transcatheter a...

there is a paucity of literature regarding iatrogenic aortic valve perforation after cardiac operation is performed in the vicinity of the aortic valve. this report describes the clinical, echocardiographic, and angiocardiographic recognition of iatrogenic aortic valve perforation in a patient who had previously under gone membranous ventricular septal defect and pulmonary stenosis. five days a...

BACKGROUND Any form of surgery or tissue damage causes release of tissue factor into the circulation. This may lead to the accelerated consumption of coagulation factors, resulting in severe consumptive coagulopathy. In this study, we compared the molecular markers involved in coagulation activation during cardiopulmonary bypass (CPB) between patients who underwent aortic replacement surgery an...
