In the late 1990s the development of endoscopic submucosal dissection (ESD) using different knives revolutionized the spectrum of endoscopic resection techniques allowing en bloc resections regardless of the lesion’s size [5] . In combination with new data on the risk of lymph node metastases in early gastric cancer resection, criteria could be extended [6, 7] . The so-called expanded criteria ...

Dieulafoy lesions, a rare cause of gastrointestinal hemorrhage, are caliber-persistent submucosal arteries without surrounding ulcers or mucosal lesions. Dieulafoy lesions rarely bleed profusely and may be missed during diagnostic endoscopy. Most of these lesions are located in the stomach; they have been reported only rarely in other parts of the gastrointestinal tract, including the small bow...

A 50-year-old woman presented with dyspepsia but no history of weight loss or gastrointestinal bleeding. Esophagogastroduodenoscopy (EGD) showed a sessile polypoidal submucosal lesion in the second part of the duodenum close to the ampulla with a central opening (fish-mouth appearance) (▶Fig. 1). The contour of this lesion was smooth and there was no disruption of the surrounding folds, nor ulc...

Submucosal chromoendoscopy involves the injection of a solution containing a vital stain, usually indigo carmine, into the intestinal wall submucosal layer. This allows to: Better delimit and characterize the various epithelia present (colonic mucosa, adenoma, hyperplastic polyp, serrated polyp, small bowel mucosa); expose and delimit lesion implantation areas; cooperate in the lifting of resec...

BACKGROUND/AIMS This study evaluated the role of a second-look endoscopy after gastric endoscopic submucosal dissection in patients without signs of bleeding. METHODS Between March 2011 and March 2012, 407 patients with gastric neoplasms who underwent endoscopic submucosal dissection for 445 lesions were retrospectively reviewed. After the patients had undergone endoscopic submucosal dissecti...

BACKGROUND The rate of occult adenocarcinoma at esophagectomy in patients with Barrett's esophagus (BE) and high-grade dysplasia (HGD) has been reported to be approximately 40%. Recently, it has been suggested that this risk may be overestimated. OBJECTIVE Our purpose was to determine the rate of submucosal invasive adenocarcinoma in patients undergoing esophagectomy for BE after biopsy diagn...

INTRODUCTION The preoperative diagnosis of submucosal lesions in the gut may be complicated. Conventional endoscopy does not allow to clearly establishing a diagnosis, and does not adequately assess lesion size. Furthermore, endoscopic biopsy is usually not diagnostic. Cytology as performed by means of fine-needle puncture does not have enough sensitivity and specificity to be considered the go...

Endoscopic submucosal dissection has made it possible to resect large lesions during a single operation. The present study was undertaken to compare the time taken for recovery from artificial ulcers after endoscopic submucosal dissection between an H(2) Receptor Antagonist treatment group and a Proton Pump Inhibitor treatment group, focusing on analysis of the time course of reduction rate in ...

Submucosal lesions (SMLs) of the upper gastrointestinal tract (1-4) represent a significant diagnostic and therapeutic challenge to both endoscopists and gastroenterologists. Such lesions may be assessed by using multifrequency transendoscopic miniprobes, or endoscopic ultrasonography (EUS), either radial or linear. Fine-needle puncture-aspiration (FNA) of SMTs should be guided by only linear EUS.

Introduction: Endoscopic submucosal dissection has been widely accepted as a standard treatment for early gastric cancers. However, endoscopic submucosal dissection for an early gastric remnant cancer post-proximal gastrectomy with jejunal interposition is not yet widespread. Case Series: A large, flat, elevated lesion was detected in an 83-year-old male (Case 1), whereas two separate flat, ele...
