The results of closed mitral valvotomy operations in 359 patients operated on from August 1957 to October 1974 were assessed at July 1975. About 60% of the patients were in good health. These results suggest that there is still a place for closed mitral valvotomy in carefully selected cases.

To evaluate the electrophysiologic changes in the cardiac conduction system that occur during percutaneous mitral or aortic balloon valvotomy, we prospectively studied the conduction system in 19 patients (10 mitral, 8 aortic, and 1 both) undergoing this procedure. A His bundle electrogram was recorded in all patients, and when sinus rhythm was present, the atrioventricular (AV) node effective ...

OBJECTIVE Aortic valvotomy is widely used for the treatment of congenital aortic stenosis in children. We sought to evaluate whether the predominant post-valvotomy physiology, aortic insufficiency (AI) or aortic stenosis (AS) independently affected patient outcome. METHODS From 1972-2002, 57 children with congenital aortic stenosis underwent valvotomy. We divided age-matched patients with res...

SURGICAL TREATMENT has increased the life expectancy of patients with mitral stenosis and has favorably modified the natural course of the disease.1-5 Hence, there is a tendency to operate on patients without serious disability in order to prevent or delay the complications of systemic and pulmonary emboli, pulmonary hypertension, congestive heart failure, and atrial fibrillation. However, the ...

BACKGROUND There have been a number of studies on mitral valve replacement and repeated percutaneous mitral balloon valvotomy for mitral valve restenosis after percutaneous mitral balloon valvotomy. However, studies on mitral valve repair for these patients are rare. In this study, we analyzed postoperative outcomes of mitral valve repair for mitral valve restenosis after percutaneous mitral ba...

BACKGROUND Parachute mitral valve (PMV) is defined as a unifocal attachment of the mitral valve chordae to a single or dominant papillary muscle and may cause subvalvar obstruction. We sought to determine factors associated with outcomes. METHODS AND RESULTS Patients (n=84; 64% male) who presented between 1977 and 2001 at a median age of 3 days (range, birth to 5.4 years) were assessed with P...

Pulmonic stenosis refers to a dynamic or fixed anatomic obstruction flow from the right ventricle pulmonary arterial vasculature. Although most commonly diagnosed and treated in pediatric population, some patients with severe, isolated PS may be for first time as adults, represents approximately 10% of all congenital heart disease. We report cases three adolescents, who presented ER after havin...

A well executed transseptal puncture is crucial for successful percutaneous transvenous mitral balloon valvotomy. We report a case of accidentally damaged Mullins sheath dilator. Due to immediate unavailability of another dilator at cardiac catheterization laboratory, angioplasty guidewire and balloon was used as an additional assisting armamentarium for successful percutaneous transvenous mitr...

308 As technology advances resulted in more effective percutaneous approaches to the treatment of valvular stenosis, the terminology used to describe these procedures evolved as well. The term “commissurotomy” originated from the surgical procedure developed for commissural mitral valve stenosis (see chapter 34), and some thought valvuloplasty should be reserved for surgical procedures that mor...
