遺伝性びまん性胃癌は,胃印環細胞癌と乳腺小葉癌を高率発症することを特徴とした常染色体優性遺伝の疾患である.今回,多発びまん性胃癌と乳腺小葉癌の既往から遺伝性びまん性胃癌を疑った症例を経験した.症例は77歳の女性で,45歳時に乳癌の手術歴がある.上部内視鏡検査で幽門前庭部から体中部小彎に多発する褪色調小陥凹を認め,生検で低分化腺癌の診断となり,多発早期胃癌の診断で胃全摘術を施行した.再建時に空腸と肝S3後面に小結節を認め診断目的に切除を行った.病理組織学的検査では胃全域に少なくとも155か所の多発癌を認めた.採取した空腸と肝臓からも同様の腺癌を認め転移と診断した.50歳未満で発症した小葉性乳癌の既往と,特徴的な胃の病理所見から遺伝性びまん性胃癌を疑った.本邦での遺伝性びまん性胃癌の報告は非常に少なく,診断および治療選択の構築と診療体制の整備のため症例集積が望まれる.
