A survey of five Nocardia spp. with respect to susceptibility towards three macrolides (erythromycin, rokitamycin, and midecamycin) showed that the Nocardia spp. have different susceptibility profiles. Most of the resistance was due to the inactivation of the macrolides by phosphorylation, glycosylation, reduction, deacylation, or a combination thereof.

A screening assay to test the inducing capacity of macrolides by fusing the attenuator of the inducible erm(B) gene from Streptococcus pneumoniae HM28 with the gfpmut1 gene has been designed. Fluorescence was detected under UV light around disks impregnated with inducer macrolides (erythromycin or azithromycin) but not with noninducer ketolides. Induction could be quantified by fluorometry.

We present the case of a 60-year old female patient, with a 10 year history of non-CF bronchiectasis and use of macrolides as maintenance immunomodulatory treatment, who was diagnosed with macrolide-resistant Mycobacterium avium complex lung disease. Macrolides' immunomodulatory effect is appealing for non- CF bronchiectasis patients, hiding a high risk for resistance emergence.
