Castleman's disease is a rare lymphoproliferative disorder. 症例は75歳,男性.膀胱癌術後で当院泌尿器科に通院中であった.定期検査でのCTで,以前より指摘されていた膵尾部の嚢胞に増大傾向を指摘された.当院消化器内科で精査の結果,膵体部に10mm,膵尾部に25mm大の嚢胞性病変を指摘され,分枝型膵管内乳頭粘液性腫瘍(IPMN)と診断された.膵体部の病変は超音波内視鏡検査で嚢胞内に5mm程度の辺縁不正な結節を認め,同部位はソナゾイド造影で比較的強い濃染を呈した.High-risk stigmataと判断し,脾温存膵体尾部切除術を施行した.病理組織学的検査では膵体部には嚢胞変性を伴う充実性腫瘍を認め,神経内分泌腫瘍(NET)G1と診断した.膵尾部の嚢胞性病変はIPMAの所見であった.術後1年9カ月が経過するが,無再発で経過している.嚢胞変性をきたした膵NENとIPMNが併存した症例は非常に稀であり,文献的考察を加えて報告する.
