OBJECTIVE Candidates for revision tympanoplasty have experienced at least one failed attempt at repair of the tympanic membrane and are, therefore, at higher risk for subsequent repair failure. The adjunctive use of mastoidectomy with tympanoplasty in those patients with noncholesteatomatous chronic otitis media is often used to decrease the risk for subsequent failure. Tympanoplasty now frequently uses cartilage to repair bone lysis in the external auditory canal (EAC) and to reinforce the tympanic membrane. Once in place, however, the cartilage may complicate ossiculoplasty by obstructing visualization of the stapedial region. Posterior tympanotomy, when included in the tympanoplasty procedure, may provide an interesting means of positioning the ossiculoplasty.

In our otological practice, we prefer sliced tragal cartilageperichondrium grafting for reconstruction of the tympanic membrane perforations. In an attempt to reduce residual and recurrent perforations in our series of cartilage tympanoplasty using sliced tragal cartilage perichondrium composite graft, we reinforce tragal cartilage with temporalis fascia in large perforations. Aim: The purpose ...
