Analysis of quality control issues in lakop sapu ijuk using Root Cause Analysis (RCA)

<p class="Abstract"><em>UD Cibinong Indah Plastik is engaged in the production of cleaning tools, but proportion defective lakop sapu ijuk 6.2% total production, whereas company's set standard for products 5%, resulting financial losses. In this study, Root Cause Analysis method was employed to identify root causes and propose improvements. Through Fault Tree Analysis, it found that most significant cause lack maintenance replacement machines. The suggested solutions through Process Decision Program Chart are improve schedule, provide additional training workers operators, enhance supervision system SOP socialization better outcomes future.</em></p>
