Application of the heart failure meta-score to predict prognosis in patients with cardiac resynchronization defibrillators

BackgroundThe Heart Failure (HF) Meta-score may be useful in predicting prognosis patients with primary prevention cardiac resynchronization defibrillators (CRT-D) considering the competing risk of appropriate defibrillator shock versus mortality.MethodsData from 648 consecutive two centers were used for evaluation performance HF Meta-score. The endpoint was mortality and secondary time to first implantable cardioverter-defibrillator (ICD) or death without prior ICD shock. Fine-Gray model regression analysis.ResultsIn entire cohort, 237 died over a median follow-up 5.2 years. Five-year cumulative incidence ranged 12% 53%, quintiles 1 through 5 Meta-score, respectively (log-rank P < 0.001). Compared lowest quintile, higher highest quintile (HR 6.9; 95%CI 3.7–12.8). had excellent calibration, accuracy, good discrimination (C-statistic 0.76 at 1-year 0.71 5-year). compared (sub HR 5.8; 3.1–11.0, 0.001).ConclusionsOur study demonstrated ability predict survival treated CRT-D as prevention. proved identifying subgroup significantly poor despite CRT-D.
