Background: Management of acromioclavicular joint (ACJ) separations depends on the type, chronicity injury, and patient demographics. Indications: Coracoclavicular ligament reconstruction may be indicated for patients with chronic type 3 ACJ separation who have failed conservative treatment. We describe an arthroscopic-assisted approach to facilitate graft passage around coracoid. Technique: Patients are placed in a beach chair position. A longitudinal incision at midline distal clavicle is utilized access ACJ, clavicle, The anterior, posterior, undersurface exposed. standard posterior viewing anterolateral working portals created. shuttling suture passed arthroscopically used pass allograft nonbiologic augmentation sutures through singular hole secured DogBone button. wrapped recreate coracoclavicular ligaments sutures. capsule reconstructed by suturing posterior/medial limb, which kept long, periosteum. Meticulous, layered closure performed particular attention closing deltotrapezial fascia. Results: postoperative x-ray 2 weeks 6 months obtained confirm proper positioning. sling discontinued weeks, supervised physical therapy initiated. At months, typically cleared full unrestricted activity. Discussion/Conclusion: Arthroscopic-assisted fail management can excellent outcomes. use arthroscopic assistance ease 70° scope helpful coracoid visualization, biologic tendon cases vital achieving good outcome.
