چکیده
BACKGROUND Identifying predictive factors for a normal outcome at admission in the labor ward would be of value planning care, timing interventions, and preventing dystocia. Clinical assessments fetal head station position start have some value, but ultrasound methods this purpose has not been investigated. Studies using transperineal before onset show possibilities these to predict outcomes. OBJECTIVE This study aimed investigate whether measurements during first examination active phase were associated with duration phases need operative delivery. STUDY DESIGN was secondary analysis prospective cohort Landspitali University Hospital, Reykjavík, Iceland. Nulliparous women ?37 weeks’ gestation single fetus cephalic presentation spontaneous eligible study. The recruitment period from January 2016 April 2018. Women examined by midwife on included if they labor, which defined as regular contractions fully effaced cervix, dilatation ?4 cm. An performed separate examiner within 15 minutes; both examiners blinded other's results. Transabdominal examinations used assess position, cervical dilatation, station, expressed head-perineum distance angle progression. Duration estimated hazard ratio delivery Kaplan-Meier curves Cox regression analysis. ratios adjusted maternal age body mass index. associations between parameters mode evaluated receiver operating characteristic curves. RESULTS Median times 490 minutes ?45 mm 682 >45 (log-rank test, P=.009; shorter distance, 1.47 [95% confidence interval, 0.83–2.60]). median durations 506 an progression ?93° 732 <93° P=.008; ratio, 2.07 1.15–3.72]). nonocciput posterior positions 677 occiput P=.07; 1.52 0.96–2.38]) 429 ?6 cm 704 4 5 P=.002; 3.11 1.68–5.77]). Overall, there 75 deliveries; among those deliveries, 16 instrumental vaginal deliveries (1 forceps ventouse deliveries), 8 cesarean deliveries. The Shape of Uterine Contractions and Labor Progress in the Spontaneous Active Labor
Background: Dystocia is the most common indication of primary cesarean section. The most common cause of dystocia is uterine dysfunction. In prolonged labor, more attention is usually paid to the fetus and pelvis rather than to the role of uterine contractions in a delivery. Therefore, we decided to determine the relationship between the labor progress and uterine contractions shapes. Methods: I...
متن کاملUltrasound in labor and delivery.
Ultrasound may play an important role in the management of labor and delivery. Induction of labor is a common obstetric intervention, performed in about 20% of pregnancies. Pre-induction cervical length, measured by transvaginal sonography, has been shown to have a significant association with the induction-to-delivery interval and the risk for cesarean section. In the management of labor there...
متن کاملEffect of self-hypnosis on the experience of delivery fear and duration of labor in primiparity: A randomized clinical trial
Background and Aims: Fear of delivery in females can lead to negative side effects, such as the experience of long and hard labor. Reduction of delivery fear is one of the goals of natural delivery program, and hypnosis has been considered a way to reduce this fear. With regard to the contradictory results of the studies in this field, this study aimed to determine the effect of self-hypnosis o...

Introduction: Prolonged labor can lead to maternal and neonatal complications. The purpose of this study was to investigate the effect of Pethidine on the duration of active labor in nulli-parous women. Materials and methods: In a randomized clinical 90 nulliparous women with uncomplicated singleton pregnancies at term were randomly assigned into two groups. The first group received 50 mg Pet...
متن کاملEffect of continuous support during labor on duration of labor and rate of cesarean delivery.
OBJECTIVE To evaluate the effect of continuous support provided by midwives during labor on the duration of the different stages of labor and the rate of cesarean delivery. METHOD A randomized trial of 100 eligible nulliparous women who had not received education classes on childbirth. In the intervention group (n=50), continuous support during labor was provided; the control group (n=50) did...
