Clinical and morphological response to neoadjuvant chemotherapy followed by chemoradiotherapy in a patient with locally advanced gastric cancer: a case report

Background . Gastric cancer is one of the most common cancers with highest incidence and mortality rates worldwide. Gastrectomy/distal subtotal resection D2 lymphadenectomy perioperative chemotherapy FLOT standard treatment for locally advanced gastric cancer. Currently, role chemoradiation therapy in has not been well determined. From 2019 to 2021, a prospective phase 2 clinical trial was undertaken at A. Tsyb Medical Radiological Research Centre (MRRC) evaluate safety efficacy neoadjuvant therapy: induction (2 cycles) chemoradiotherapy (total dose 46 Gy concurrent capecitabine oxaliplatin) followed by surgery. The aim study demonstrate effectiveness favorable pathological response, downstaging more than 2.5-year disease-free survival. Case description A 48 year-old male patient admitted MRRC complain epigastric pains months. Gastroscopic biopsy revealed comprehensive examination proximal involving abdominal segment esophagus (cT3N3aM0, stage III). invited participate trial. accepted voluntarily signed an informed consent. He received cycles surgery (gastrectomy lymph node dissection). Histological surgical specimen grade 1b response (Becker criteria). nodes complete response. alive after 33 months gollow-up without evidence disease progression. Conclusion Neoadjuvant resulted
