患者:1歳 9カ月,タイ人の女児.血液型 0型 Rh (+). 主訴:黄疸,吐血.家族歴:特記すべきことなし. 現病歴:1998 年 4 月 23 日,38 週,2,620 g で帝王切 開にて出生.生後高ビリルビン血症にて光線療法を 21 日間施行した.生後 1カ月より眼球結膜の黄染を認め, 2カ月目には血清総ビリルビン値 10.0 mg dl に上昇 した.7月 1日(日齢 69 日)に,先天性胆道閉鎖症の 診断の下,葛西手術を施行し,術後肝機能は改善し, 血清総ビリルビン値も 6.0 mg dl まで低下した.平成 11 年 12 月 31 日に突然吐血をきたし,さらに,肝機能 の悪化,血清総ビリルビン値の上昇を認めた.先天性 胆道閉鎖症,食道静脈瘤の診断の下,2000 年 1 月 13 日,肝移植目的にて東京大学付属病院に入院となった. その後,食道静脈瘤に対し結紮療法を施行...
